DENTAL PATIENTS ARE SEEKING NEW WAYS TO RECEIVE AFFORDABLE DENTAL CARE

  • This program is offered to individuals and families not currently enrolled or utilizing any other dental programs AND you must have a ZERO BALANCE TO ENROLL OR RE-ENROLL

    $300.00 - Adult Membership
    $252.00 - Additional Adult Family Member (non-perio patient)
    $225.00 - Child Membership (to age 14)

  • YEARLY MEMBERSHIP INCLUDES:

    • Two dental encounters in a twelve month period (exams)
    • Two "gum therapy" or preventive prophylaxis in a twelve month period
    • Additional "gum therapy" appointments within the twelve month period - 15% discount applies to procedure(s) fees*
    • Appropriate x-rays and/or necessary diagnostics (CariVu), photos
    • 15% discount* on any "in-office" dental treatment, including any periodontal treatment
    • Lab fees not included
    • Lifetime stipulation (while on plan) one full mouth x-ray (4-18 rads)/one initial comprehensive exam
